Evidence Submission Forms

Does anybody know where I can find pictures of the evidence submission forms they use on the show? I was just wondering cause I wanna compare them to some others.

You could try looking for a really clear shot of one from one of the eps, then asking for the screencap in the fanart thread? Or just asking your question in that thread maybe?

I am sending this over here since it has to do with evidence having to do with forensic's etc. I would suggest asking in Fan art, but this might be a good place to, to talk about them.
 
Since I do not have the DVDs of the show and haven't found the forms for the Las Vegas PD on the internet yet, here are some forms from other cities or states. Some of them are in PDF format for use with Adobe Reader.
