Mac took his wedding ring off in the finale of S1. A year would have been since the beginning of S2. Maybe that's why Mac was so happy all of S2.

Also, there could possibly be other marks on the bullet created afterwards- like in "Creatures of the Night" on CSI: NY. It may not be common, but I think it could be hard to determine the gun marks from other marks.

And honestly, I don't think she'd be secretive about killing in self defense. It would for sure be on her file. I think whatever it is, she wanted to get away from it, from her past.
