"Happy Ending" -- Incredible arrow

Hey anyone confused by the arrow hit Happy? the wound is a pair of round small holes on the neck, indicating a through-and-through. But the weapon arrow actually got three big "feather" (actually they are called "wings" technically) at the end of it. so the wound shall be a pair of BIG holes if the whole arrow pass through. How do the script writer think about that?
